Full Job Description
Working with a diverse Multidisciplinary team, providing holistic assessments and treatments to our elderly cohort. A busy community assessment unit, accepting patients from A&E, community and acute wards. Focus on safe/effective discharge planning reducing the risk of readmission. We are looking to recruit a highly motivated, enthusiastic staff nurse to work as part of our multidisciplinary team. It is a full time role (37.5 hours) across Monday to Sunday. The Community Assessment Unit is run by a team of dedicated specialist community practitioners who are experts in providing the most appropriate care for our frail & elderly population. Applicants will be required to see patients and families face to face within a community setting. Therefore it cannot be ruled out that they may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to ask patients with symptoms to cancel appointments. An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks. For overseas candidates there is an alternate route into the NHS as only those with active (or pending) NMC registration will be considered. Duties and Responsibilities Key results from the job holder To deliver care to 21 patients in the Community Assessment Unit comprising 9 single rooms and 3 4-bedded bays primarily operating as an older persons assessment unit assessing for step up care and discharge into the community. The availability of the single rooms will enable the provision of care to infectious patients who require isolation. Planning and Organisational Duties
Accountable for nursing practice and takes every opportunity to sustain and improve his/her knowledge and professional competence.
Planning and managing care under the supervision of other members of the disciplinary team following diagnostic pathways and utilising community specialists
Participates in the assessing, planning, implementing and evaluating programmes of care relevant to patients within sphere of responsibility and under supervision in the community assessment unit
To be sensitive to the needs of patients in relation to courtesy, privacy and dignity.
To be familiar and adhere to Trust and Divisional objectives, policies, guidelines and current legislation.
Communications and Key Working Relationships
Communication with all key members of the multidisciplinary team
Responsibility for Human Resources
Follow all WWL policies and procedures
Ensure mandatory training is up to date
Responsibility for Teaching
Responsible for teaching any students on the Unit
Responsible for orientating new starters
Responsibility for Health & Safety
Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
